Notes to Entry Procedure:
Application must reach the NGA office by 5 pm on the date indicated.
Payment must be received with the application in order to be accepted.
Tournaments are open until stated deadline.

Age Limits:
Nebraska Senior Amateur - must be 50 on or before TBA.
Nebraska Senior Match Play - must be 50 on or before Aug. 6, 2012.
Nebraska Mid-Amateur - must be 25 on or before Sept. 8, 2012.

* Junior golfer is defined as an amateur golfer who has not yet reached the September 1 following graduation from secondary school.
